In Office Dental Plan

In an effort to serve you in a new and different way, our office has created our In-Office dental membership plan. This is not dental insurance. This program is offered to our patients who do not have dental coverage.

The In-Office dental plan is an alternative for families or individuals, who want excellent dental health coverage without the barriers of traditional dental insurance:

• No high costs – No waiting periods – No deductible
• No annual maximums – No pre-existing conditions exclusions – No hassle dealing with insurance companies

ELIGIBILITY: Anybody is qualified to apply for our In-Office Dental Membership Plan. Participants are eligible upon payment of the enrollment fee. This plan is administered solely by this dental office and may be discontinued at the end of any month, with or without notice.

• Yearly $330 and additional family members $275
• Monthly $33 and additional family members $23

• 1 Panoramic x-ray (1 per 5 years)
• Half of the Whitening Custom trays and the first set of whitening
• 2 Periodic oral evaluation
• 1 Emergency Exam

• 1 Fluoride for 18yrs or younger

• $1,000.00 Off Invisalign (full comprehensive)
